All yellow medical forms should now be with your group leader. You need a form for every activity out of school and so may need an additional form; these are available outside the Classroom Assistants’ office in Bruntsfield House.   If you are doing an in school activity, you should still hand one form to the group leader.
KIC Medical forms are available from Welfare.
